UI of pro forms broken- Need assistance ASAP

Our payment forms look like this on our site:

When they should look like this:

I have been working with the host to see if they could pinpoint the issue and this was all they could provide:
I looked up the troubleshooting page of the s2member plugin, but it did not have any useful information as only plugin/theme conflict was suggested in it.

Via the browser console I noticed that certain s2member scripts are failing to load, which is most likely what causes the issue.

Lastly, I tried to disable the .htaccess file in case there is a rule that interferes with the functionality of the s2member plugin, but there was none.

Hi Kim.

Yes, the form looks like that because the s2member-o.php didn’t load. That file serves s2’s JS and CSS.

It’s not a problem with how s2Member works, there’s something outside of s2Member preventing access to that file.

You need to find what is blocking it, e.g. a permission on your server, or some setting on CloudFlare, or something.

Thank you much, this got them to find the issue which was a rule in our htaccess file reverting subfolders to outdated PHP versions.
