Do you mean the s2Member-Profile shortcode form? https://s2member.com/kb-article/s2member-profile-shortcode/

There isn’t a template file for it. You’ll find its code here: s2member/src/includes/classes/sc-profile-in.inc.php

There you can see several hooks you can use. You could use the filter ws_plugin__s2member_sc_profile to customize it before it gets returned.

The cancellation form is s2Member’s, but the rest I don’t recognize.
