Remove Checkout Options

I created an Authorize.Net Pro form that you can see here - http://ycnmediagroup.com/bi-monthly-coaching/

A few issues:
I, I don’t offer any Checkout options (I don’t know what that field needs to be because it shows up empty). So how do I remove it?

  1. When I enter a coupon code and hit Apply, nothing happens. It only applies the code when I hit submit.

  2. My coupon code is for 100% off, but even after it’s applied I still see the credit card fields and need to add them.

  3. as you can see, no credit card icons are showing under the billing method. Only the radio boxes.
    how do I fix that?

There is a Failed to load resource: the server responded with a status of 403 (Forbidden) error message in the Developer console for that page which shows that s2member-o.php is not being loaded. That’s most likely the reason the form is not functioning correctly. This is most likely a permissions error on your server.

That said, this form isn’t going to work anyway because I’m 99.99% sure Authorize.net requires an https connection.

Second issue, what is the Card Start Date (mm/yyyy), or Issue Number * and how do I remove it?
