i do not have two forms on my page… and still have this problem… please help!!! (fyi the link to article above is 404)

Yeah, this bug must have reintroduced at some point - I see when I have a single s2Member Authorize.NET Pro form embedded in a WordPress custom template that gets fired by do_shortcode in PHP. Definitely didn’t show that extra card field in the past.

It could be caused by a plugin/theme affecting s2’s javascript for the pro-form. You can test for conflicts deactivating other plugins or the theme one by one, checking after each if the behavior goes away. Once you identify what introduces the conflict, it’s easier to go about sorting it out.
