Adding City and State to Registration Page

Tried adding a Custom Registration/Profile Field in S2 Member Pro settings. But when I tried to see if it worked I didn’t get the option on Subscribe here form?<imgsrc="/uploads/default/original/1X/fc3b8cb287712a1452bec393d6c81543b924fa85.png" width=“690” height=“388”>

Can you guide me where or how to enter in the correct codes for the form? I want to be able to see from what city and state my customers are registering from.

I haven’t received any information that can help me with this problem. Is there any one out there that knows why when I make a change in S2Member Pro why the changes don’t appear?
